A Facilitator’s Guide to the Conversations That Reveal Root Causes

“Communication is terrible.”
A leadership team once told me, “Tammy, we’ve been working on this communication issue for almost four years.” They had rewritten procedures, held more meetings, provided more training, created action plans, and sent follow-up emails. Yet nothing changed. About forty-five minutes into our facilitated conversation, someone quietly said, “I don’t think communication is actually our problem.” Another leader added, “I think we just don’t know who’s supposed to make decisions.” Everything shifted. Communication wasn’t the problem—it was simply where the real problem showed itself.
Stay Curious a Little Longer
One phrase you’ll hear me say often is, “Let’s stay curious for another minute.” Curiosity creates understanding. Understanding creates better decisions. Better decisions create better results.
Why “Why?” Doesn’t Always Work
Depending on how it is asked, “Why?” can sound like blame. Instead, invite curiosity with questions like:
- Tell me more about that.
- Help me understand.
- What contributed to that?
- Walk me through what happened.
- What were you noticing?
- What else might be true?
The Five Levels of Understanding™
Level One – The Event
What happened? Separate observations from opinions.
Level Two – The Experience
What contributed to it? Explore perspectives and conditions.
Level Three – The Pattern
Where else do we see this? Patterns reveal systems.
Level Four – The System
What about our system allows this to continue?
Level Five – The Purpose
Why does solving this matter?
The Conversation Beneath the Conversation
Leader: “We have a communication problem.”
Facilitator: “Tell me more.”
Leader: “People don’t know who is making decisions.”
Facilitator: “Why is that?”
Leader: “We’ve never defined decision ownership.” The conversation didn’t become more complicated. It became more accurate.

After someone answers a question, wait five more seconds. Don’t fill the silence. Almost every time, someone says, “Actually…” That’s usually when the real conversation begins.

Final Thoughts
